Case Manager

Responsibilities:
Provide case management services to clients, including assessment, planning, coordination, monitoring, and evaluation of services.
Develop and implement individualized service plans for clients.
Provide crisis intervention and support services.
Provide advocacy and support to clients.
Maintain accurate and up-to-date client records.
Collaborate with other professionals to coordinate services for clients.
Provide referrals to community resources.
Participate in team meetings and other professional development activities.
Qualifications:
Bachelors Degree required. Degree in Social Work, Psychology, Counseling, or other related social service field preferred.
Minimum of 2 years of experience in case management.
Valid drivers license required.
